The deadline for employees in the medical plan and their covered spouse/partner to complete the Annual Health Assessment is Monday, Dec. 15. Click here for instructions [PDF] on the process. Note: If you or your covered spouse/partner is new to the plan, you will complete your assessment in January.

Complete the Assessment Before Dec. 15
The deadline, unless a new participant, is Monday, Dec. 15. This deadline will NOT be extended. You will get a follow up email from Benefits within two weeks of completing your assessment to confirm we have been notified of your completion.
